These Terms of Service ("Terms") govern your use of moderndentalhub.com (the "Site"), which is operated by Modern Dental Smile PC, doing business as Modern Dental Hub ("we", "us" or "the practice"), a dental practice at 235 Sunrise Highway, Merrick, NY 11566. By using the Site you agree to these Terms. Our Privacy Policy explains how we handle information collected through the Site and forms part of these Terms.
Using this website
You may use the Site for personal, non-commercial purposes: to learn about our services, check our hours and insurance information, contact us and request an appointment. You agree not to:

No doctor-patient relationship through this site
Reading the Site, sending a message or requesting an appointment does not create a doctor-patient relationship. That relationship begins when you are examined by one of our dentists at the practice. Please do not send detailed health information through website forms; we will discuss your health history with you in person or by phone.
Appointment requests and online booking
- A request submitted through the Site's forms, or through the "book online" link, is a request, not a confirmed appointment.
- Your appointment is confirmed only when the office confirms it with you by phone or email.
- The "book online" link opens our appointment request form, hosted for the practice by its technology provider. Choosing a service, a day and a time there sends a request; the office contacts you to confirm a time.
- To cancel or change an appointment, call the office as early as you can.
Forms you submit
When you submit a form on the Site, you confirm that the information is accurate and that you are the person it describes, or are authorized to act for that person, such as a parent or guardian. By giving your consent on the form, you agree that we may contact you at the phone number or email address you provide in response to your request. You can withdraw that consent at any time by telling us.
Intellectual property
The text, images, logos, graphics and design of the Site belong to Modern Dental Smile PC or are used with permission. You may view and print pages for your personal use. You may not reproduce, distribute, modify or use the Site's content, the name Modern Dental Hub or the practice's logo for commercial purposes without our written permission.
Third-party links and services
The Site links to services operated by other companies, including Google Maps. We do not control those services and are not responsible for their content, availability or privacy practices. Your use of those services is governed by their own terms.
