Form submissions are sent directly to Logika, the practice's secure form service, which processes them on the practice's behalf as a service provider and makes them available to our team.
Please do not include detailed health information in a website form. A short description of what you need, such as "toothache" or "new patient exam", is enough; we will discuss the details with you by phone or in person.
The "book online" link on this site opens the practice's appointment request form, hosted for the practice by Logika, the platform it uses for patient requests. What you enter there (the service you need, a preferred day and time, your name, phone number and email, and optionally your insurance) goes to the practice so the office can contact you and arrange your visit, and is handled as described in this policy.
Information collected automatically
- Google Analytics. On moderndentalhub.com we use Google Analytics, loaded through Google Tag Manager, to understand how the website is used: which pages are viewed, how far people scroll and which buttons are used, for example a tap on the phone number or on the booking link. Google Analytics sets first-party cookies (such as _ga) that tell browsers apart. It never receives your name, phone number, email address, messages or anything else you type into a form. Advertising personalization and Google Signals are turned off, and we do not use this information for remarketing or advertising. Google processes this information under its own terms. You can block these cookies in your browser settings or with Google's opt-out add-on.
- Where you came from. If you arrive from an advertisement or another website, the site remembers that source in your browser for the current tab only. When you tap the phone number, open the booking link or start a chat, the source and the button you used are passed to the patient-communication system we use (Logika), so we know which channel brought you. This information does not include your name or contact details, and it is not shared with advertisers.
- The web server that delivers these pages records standard technical logs (IP address, browser type, page requested, date and time), which we use only to keep the site running and secure.
How we use your information
