These SMS Terms & Conditions apply to text messages sent by Modern Dental Smile PC, doing business as Modern Dental Hub ("Modern Dental Hub", "we", "us"), a dental practice at 235 Sunrise Highway, Merrick, NY 11566.
Program description
Modern Dental Hub sends text messages from (516) 855-6855 to people who agree to receive them. Messages are about your request and your appointments: replies to the message or callback request you sent through our website, confirmations and reminders of the visits we schedule with you, and practical updates about them. We do not send marketing or promotional text messages under this program, and we do not send treatment details or other health information by text.
Consent
You agree to receive these messages by checking the optional text message box on the contact form or the callback form on moderndentalhub.com. The box reads "I agree to receive text messages from Modern Dental Hub about my request and my appointments. Message frequency varies. Message and data rates may apply. Reply STOP to opt out, HELP for help." It is never checked in advance, and agreeing is not a condition of any appointment, treatment or purchase. We record the phone number, the time, the wording shown and the page where you agreed.

Message frequency varies with your request and your appointments. Message and data rates may apply, depending on your mobile plan. Carriers are not liable for delayed or undelivered messages.
Opting out and help
Reply STOP to any message to stop receiving text messages; we will send one final confirmation and then stop. Reply HELP for help. Your information
We use your mobile number and your consent only to send the messages described above and to keep records of your consent. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. Service providers that deliver messages for us receive only what they need to deliver them.
